Impact of Behaviour Incident Report for Clubs

Creating A safe space to play, explore and grow

As soon as possible, this report is to be completed by the member of staff who witnessed and/or dealt with an incident where a child has been impacted in some way (for example, targeted, bullied, ostracised or injured by another child). Once completed the Club Manager (or deputy) should check the details before submission.

Information to note: –

  • Once completed and submitted this report will be received by the Office Administrator, saved as a PDF and placed in a folder in SharePoint>In 04. Clubs> Behaviour Management>Setting.
  • Clubs will need to create a folder for the child and save this report and any other supporting information to that folder. Please use the format – child surname, child first name & date of incident xx.xx.xxxx. This will allow the Manager and the Operations Director access to all relevant information.
  • The Operations Director will receive a copy of this report
  • In the event of a safeguarding issues or serious injury, the impact of this incident may need to be reported, and details passed on to relevant authorities (Ofsted, RIDDOR, LADO, MASH, Insurance Company). Speak to the Club Manager or refer to the ‘External Authority Reporting Guidance’ for information on this.

Guidance – This report is to be completed by the staff member who dealt with the incident. Please notify the Club Manager of the incident as soon as possible who will inform and discuss with the Parent/Carer at collection, unless this was an incident that involved unacceptable behaviour, or an injury involving a head bump or biting. In this case, Parents/Carers are to be contacted immediately.

Guidance – Record the facts of the incident impact. Do not express opinions and avoid making assumptions. A Parent may subsequently request the information that you record here. Record all information so that it can be readily shared with them. Give a detailed account of what happened including the sequence of events, actions taken at the time to manage the incident and note any relevant conversations. Outline the involvement of other staff and children, Parent/Carers, or visitors, but, wherever possible, refer to another other person/child as X. Where multiple individuals are involved, for data protection purposes, use their initials only. Please refer to the training pack for examples of good practice when recording incident impact details. Use the Incident Report to record details of the child demonstrating the challenging behaviour
Was the child injured?
Guidance – If there were any physical injuries, please also complete an Accident form and follow the process for informing the child’s Parent/Carer. If there were serious injuries, this will also need to be reported to the correct authorities. Please refer to the Club Manager for guidance or refer to ‘External Authority Reporting Guidance’ and note any action taken in the follow up actions section at the end of this form.
Guidance – Please arrange for any photos regarding the impact of this incident to be uploaded to a folder created for this child in SharePoint (child’s surname, child’s first name, date of incident xx.xx.xxx). This can be arranged by the Club Manager. Note the details in the answer box above
Guidance – In all incidents inform/discuss with the Parent/Carer. Inform Parents/Carers about this incident when the child is collected. However, if the incident resulted in serious injury to this child or involves a bump to their head or being bitten, their Parents/Carers should be notified immediately. The Club Manager should be made aware immediately if an incident has resulted in injury.

Guidance –If this is a safeguarding concern, a Report of Concern must be completed, and this incident must be reported to the correct authorities. Please refer to the Club Manager for guidance or refer to ‘External Authority Reporting Guidance’.

Next Steps

  • Complete Accident Report or Report of Concern where necessary and note this in the additional information box below (These reports will ask for details of any necessary action taken to report details to external authorities as instructed in the ‘External Authority Reporting Guidance’)
  • Submit the Report
